As I understand it, Google doesn't really say "work X hours per week", it says "work whenever you want, just get this project done by [deadline]" and the deadline doesn't like to be moved just because you're sick. So it wouldn't be a matter of lack of sick days to take, just lack of time to take a sick day and still get stuff done.
